Home and Community Services (HCS) is currently recruiting for a temporary In-Home and Residential Case Manager (Social Service Specialist 3) and will be based in Aberdeen, WA.

Some of what you'll be doing:

  • Conducting and completing face to face comprehensive assessments and reassessments at the client's residence (in-home, hospital, nursing home, residential facility, etc.) using a laptop computerized system to determine client level of care, including assessing and determining nursing facility level of care needs.
  • Developing initial and ongoing care plan.
  • Providing case management services to clients meeting criteria set forth in Aging and Disability Services Long Term Care manuals and policy.
  • Assessing and providing case management to adults at high risk of institutionalized or to those adults whose needs for case management is part of a service plan developed following an adult protective service investigation. What we are looking for:

  • Knowledge of Medicaid terminology, regulations, and policies.

  • Ability to analyze a variety of programmatic issues/ problems and develop workable solutions.
  • Knowledge of payment systems (MMIS, SSPS, etc.) and service authorization procedures.
  • Ability to use and learn software applications (e.g. Word, Outlook, and Internet)
  • Must be able to complete assessments in the homes and residence of individuals.
  • Ability to communicate and comprehend through oral and forms of communications with management, peers, clients and the public. Required Qualifications:

  • One year of paid experience completing comprehensive assessments for vulnerable populations to include children, elderly or disabled populations.

  • One year as a Social Service Specialist 2 OR

  • A Master's degree in social services, human services, behavioral sciences, or an allied field AND two years of paid social service experience equivalent to a Social Service Specialist 2. OR

  • A Bachelor's degree in social services, human services, behavioral sciences or an allied field AND three years of paid social service experience equivalent to a Social Service Specialist 2. Note: A two year Master's degree in one of the above fields that included a practicum will be substituted for one year of paid social service experience.

Note: Employees must successfully complete the formal training course sponsored by their division within one year of their appointment.
