Primary Care – Mental Health Integration (PC-MHI)

The Nurse Practitioner (NP), functions as an integral member of the Primary Care Mental Health/Emergency Department Mental Health (PCMH/EDMH) team, and is responsible for providing comprehensive mental health services to patients seen in the outpatient Primary Care clinics of the VA Ann Arbor Healthcare System. The NP’s responsibilities also include timely evaluation and management of patients presented acutely to the Urgent Care/Emergency Department (as well as OP MH Clinics as needed)

The NP works collaboratively in a multi-disciplinary environment to assess, plan treatment, and implement the care of patients with acute and chronic health conditions focusing on mental health.

In addition to providing direct patient care (face to face as well as Clinical Video Conferencing) including prescribing medications (and administering injections with appropriate training), the NP provides patient and family teaching, staff education, and; consults and collaborates with other health care providers; participates in nursing and psychiatry program evaluation projects/collaborative research and ; refers patients to appropriate specialty care.

The NP promotes wellness and collaborates with patients to establish realistic health goals.

The NP demonstrates leadership in delivering holistic care through collaborative strategies including working closely with the Mental Health Service and Ambulatory Care Service staff as well as family members of patients.

The NP serves as an expert practitioner, executing position responsibilities that demonstrate leadership, experience and creative approaches to management of complex patient care.

There may be other duties as assigned including but not limited to providing back-up clinical coverage in other settings such as the Hospital (e.g. AIMH, CL, SUD-IOP) or Community Based Services (e.g. VEAR, MHICM)

Tour of duty: Usually tour is Monday - Friday, 8:00 a.m. to 4:30 p.m. but may include evening hours and weekends based on clinical need.

  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • MSN with major as a nurse practitioner
  • Full and current national board certification as a nurse practitioner from a VA-approved certifying body, which must be in the specialty to which the individual is being appointed
  • Full and current national board certification as a Nurse Practitioner from a VA-approved certifying body, which must be in the speciality to which the individual is being appointed or selected
  • BLS and ACLS certification GRADE DETERMINATIONS: Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.

The criteria are as follows:

  • Practice: practice, ethics and resource utilization;
  • Professional Development: performance, education/career development;
  • Collaboration; collaboration, collegiality;
  • Scientific Inquiry: quality of care, research.

Nurse Practitioners and Clinical Nurse Specialists. Registered nurses appointed or otherwise moving into these assignments must meet and maintain the following additional qualifications:

Nurse Practitioners. A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master’s degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. [The certification must be in the specialty to which the individual is being appointed or selected.]

Physical Requirements:

The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. AAVAHCS is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, with visits to the Emergency Department and admissions for medical co-morbidities. This position requires the ability to perform the following physical standards & environmental factors: Heavy Lifting, 45 pounds & over; heavy carrying, 45 pounds & over; straight pulling (4 hours); pushing (4 hours); use of fingers; both hands required; walking & standing 8-12 hours; repeated bending 8 hours; both legs required; ability for rapid mental & muscular coordination simultaneously; near vision correctable @ 13” to 16” to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; depth perception; ability to distinguish shades of colors; hearing (aide permitted). Working closely with others; & protracted or irregular hours of work. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
