POSITION SUMMARY: Provides overall coordination in the delivery of services and discharge planning for specified patient population. Performs concurrent analysis of patient records for purposes of utilization review and quality assurance. Promotes a cooperative and supportive relationship as liaison with patient, family, facility staff, physicians and funding representatives. Ensures continuity in the delivery of services across St. David’s entities.

REPORTS TO: Director of Case Management

SHIFTS: Part Time (16 hrs) - 8 hours Saturday and 8 hours Sunday


EDUCATION: (Required) Registered Nurse with clinically focused work history.
/ / / / EXPERIENCE /: / Minimum three years clinical experience within an acute health care setting preferred . Prefer previous recent work history in acute care case management role. Working knowledge of case management philosophy/process/role, needs assessment, principles of utilization review/quality assurance, discharge planning, and reimbursement structures (i.e. Medicare, Medicaid, HMO). / / / / KNOWLEDGE, SKILLS AND ABILITIES: Ability to communicate effectively, read and comprehend complex clinical and financial data; organize, prioritize and manage time efficiently. Utilizes computer keyboard extensively to obtain and transmit patient financial, medical and psychosocial information of a confidential nature. Operates printer to obtain computer-generated information. Utilizes telephone and FAX machine. Must be able to communicate effectively with a wide range of hospital personnel, physicians, patients/family members and representatives of outside agencies. Ability to work with patients of adolescent, adult and geriatric age.

  • Must be able to demonstrate understanding of HCA’s and St. David’s “Patients First” safety initiative by strict compliance to all safety protocols and procedures.

  • Recognizes that variables such as age, race, nationality, culture, disease-causing vulnerabilities, socioeconomic factors affect the patient populations under their care as defined in the unit’s provision of care statements. Uses interventions appropriate to these factors (e.g., interpreter, dietary preferences, social service consult, etc.) //
