          Intake Coordinator
          Department: Care Management
          Schedule: Full-time
          Shift: Evening shift
          Hours: 3p-11p Monday - Friday
          Job Details: - Licensure Required
          - 1-2 years experience is required

          JOB PURPOSE OR MISSION: To improve the quality of life of patients and families, by coordinating care through assessment, planning, implementation and evaluation as appropriate for the age of the population served, as defined in the department’s scope of service. To ensure the timely and seamless clinical transition of patients through medically appropriate levels of care.

          EXPERIENCE REQUIREMENTS

          2 years Nursing experience, Case Management preferred.

          EDUCATIONAL REQUIREMENTS

          Louisiana RN License.

          SPECIAL SKILLS AND KNOWLEDGE REQUIREMENTS

          Excellent written, verbal and listening communication skills.

          Ability to manage conflict, stress and multiple simultaneous work demands in an effective professional manner.

          Ability to work independently while collaborating with other team members.

          Ability to organize and prioritize work for optimal results. Excellent analytical and problem solving skills.

          Knowledge and ability to manage multiple patients of cultural diversity and low literacy issues in care provision.

          Knowledge of Interqual criteria.
