Incumbent provides clinical care including lab services as an Licensed Practical Nurse (LPN) at a Community Based Outpatient Clinic (CBOC) under the Aleda E. Lutz Veteran Affairs Medical Center, Saginaw, Michigan and may be requested to provide coverage at another CBOCs or at the Medical Center in Saginaw. The LPN/Licensed Vocational Nurse (LVN) and provides a wide range of clinical services to the Veteran that demonstrates complex technical judgment under direct supervision of a registered nurse or Medical Doctor. The Patient Aligned Care Team (PACT) LPN is assigned to one PACT team and panel of patients. The LPN utilizes basic nursing skills and techniques, of varying degrees of complexity, in a general care setting, to provide personal and general nursing care to adult patients, aged 18-85 and older, who require treatment for common medical conditions. In addition, the LPN utilizes Clinical Video Telehealth equipment to facilitate primary care remote visits. The LPN participates in data collection, implementation, and evaluation of care for all aged patients treated. In care of the elderly, planning and implementation is aimed at providing care to support and reduce the effects of the aging process, health maintenance and promotion, minimizing adverse effects of drugs, and maintenance of optimal psychosocial functioning, including assisting with the management of confusion and disorientation. This nurse will travel occasionally to a nearby CBOC to cover lab and/or patient care; thus a valid driver's license is required for this position. Duties if this position include, but are not limited to:
Collects and processes lab specimens per Licensed Independent Practitioner (LIP) and/or nursing standing orders.
Administers medication accurately and promptly under the general supervision of a registered nurse or LIP. Demonstrates understanding of the effects and/or reactions caused by medications, treatments, and procedures. Records and reports these observations.
Performs basic physiological measures as ordered and PRN (e.g., temperature, pulse, respirations, blood pressure, pain, diabetic foot screen, height/weight, Nx I&O;). Demonstrates ability to make astute observations and thoroughly documents care and patient response. Assists the Registered Nurse (RN) in evaluating changes in patient condition.
Obtains, records and reports objective and subjective data relevant to a historical biopsychosocial profile using Computerized Patient Record System (CPRS) templates, notes and reminders and medication reconciliation notes. Demonstrates expertise in patient observation and reporting.
Administers influenza, pneumococcal and tetanus and adult vaccinations per protocol.
Performs testing as ordered: Glucometer, Holter, Electrocardiogram (EKG), pulse oximetry, skin tests.
Work Schedule: 7:30 AM to 4:00 PM or 8:00 AM to 4:30 PM (20 hours per week, Monday through Friday, may be required to work some evenings; Saturdays are as needed)

Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.

Licensure. Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English.

Experience. Completion of at least one year of qualifying experience at the GS-5 level or equivalent and fully meeting all performance requirements for the GS-5 LPN/LVN.

Knowledge and ability to recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed.
Knowledge and understanding of human behavior, patient motivations and reactions to situations, and ability to appropriately utilize this knowledge in working effectively with patients, family members, and other staff.
Establishes constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills. Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure. Serves as a preceptor in orienting, educating, and training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures.

Note: Must have Basic Life Support (BLS) certification prior to assuming clinical duties. The BLS requirement is satisfied by successfully completing an American Heart Association (AHA) BLS Healthcare Provider Course or Military Training Network (MTN). Physical demands include repetitive body movements including the following: frequent reaching, standing, walking, and lifting of equipment and/or patients to provide patient care. This position requires extension of the hand(s) and arm(s) in any direction including above or below shoulder level, during the provision of patient care, including retrieval of needed equipment, supplies and to make contact with the patient. This position requires the maintenance of an upright position to perform duties associated with patient care. This position is typically non-sedentary, and frequently involves being in the standing position. This position requires moving about on feet to accomplish tasks, such as assisting with patient ambulation and moving one patient area to the next. This position requires the raising of objects from a lower position to a higher position or the movement of objects horizontally position to position. Lifting may be done to move a patient from one location to the next, i.e.- from the exam table to the chair, and in the movement of equipment or supplies. This position requires exerting up to 50 pounds of force occasionally and/or up to 25 pounds of force frequently to move objects. This position requires writing or otherwise working with fingers and/or hands, including one or both hands frequently. This is done when taking a patient's pulse, utilizing a stethoscope, administering medications, entering information into the computer, etc. This position requires substantial movements (motion) of the wrists, hands, and/or fingers, as is done when entering patient notes into the computer.
