The Registered Nurse (RN) Care Coordinator at the Lake Jackson CBOC provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals; this includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. The nurse's primary commitment is to the patient, whether an individual, family, group, or community. The Registered Nurse duties include but are not limited to the following:

  • Maintain current knowledge of trends and issues in health care

  • Communicate and work effectively with all levels of staff throughout the facility.

  • Provide active involvement on a Nursing or Medical Center Committees

  • Lead and coordinate interdisciplinary groups and report outcomes

  • Develop, implement and monitor performance improvement plans as identified

  • Monitor and report effectiveness of change outcomes

  • Participate in evidence based research activities annually.

  • Correspond and report in an accurate and timely manner.

  • Utilize sound clinical judgment in making decisions.

Work schedule:Monday – Friday; 8:00 am – 4:30 pm, possibility of overtime and weekends.

  • Physical standards - All applicants must meet the physical standards for the position. A physical examination prior to employment may be required.
  • Registration – Current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The RN must maintain a current, full active and unrestricted registration to continue employment with the VA
  • 2 years of nursing experience, preferred
  • Educational requirements can be met as follows:

(1) Graduate of a school of professional nursing approved by the appropriate State agency, and accredited by one of the following accrediting bodies at the time the program was completed: a) The National League for Nursing Accrediting Commission (NLNAC); or b) The Commission on Collegiate Nursing Education (CCNE). OR

(2) The completion of course work equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. OR

(3) In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration, will meet the requirement of graduation from an approved school of professional nursing.

The following requirements are preferred for this position:

  • Two or more years of nursing experience preferred.

PHYSICAL REQUIREMENTS:The physical requirements of this position include heavy lifting heavy lifting, 50 pounds heavy carrying, 50 pounds straight pulling (1-2 hours); pushing (1-2 hours); walking (2-4 hours); standing (2-6 hours); use of fingers; both hands required; ability for rapid mental & muscular coordination simultaneously; near vision correctable; far vision correctable; depth perception; Hearing (aid permitted).
