*JOB SUMMARY: *The Prospective Payment System (PPS) Coordinator performs the functions of assessing, planning, implementing and evaluating care for complex patients in areas of Rehab. She/he has extended education and experience and clinical expertise in her/his area of specialization. The components and defining characteristics of the position include clinical practice, leadership, consultation, education, research and professional development. She/he meets standards of care and practices. Promotes collaboration with physicians and personnel of other departments; demonstrates knowledge and skills necessary to provide care appropriate to complex patients served. Additionally, the Rehab PPS Coordinator demonstrates knowledge of principles of growth and development over the life span; possesses the ability to assess data reflective of the patient’s medical status and functional abilities; and interprets appropriate information needed to identify patient requirements relative to the patient’s age-specific needs. The provided care needed as described in department policies and procedures. She/he completes a competency skills checklist within 3 months of employment.
PPS Coordinatorfacilitates for rehabilitation amongst all interdisciplinary team members the initial, concurrent, and discharge assessment of care that is based upon the Patient Assessment Instrument (PAI) attends team conferences. Functions autonomously as clinician, educator, and consultant in ensuring compliance in completion of the Patient Assessment Instruments according to the standards as well as the timely submission according to CMS/State guidelines. Collaborates with the Rehab NursingManager, Rehab TherapySupervisor and interdisciplinary personnel in the planning and delivery of patient care to age and need specific patient population. Utilizes research, performance improvement data, and UDS/PAI outcome data to support and enhance patient care outcomes through program development and/or improvement. Performs and assures facility compliance by performing audits of PAI documentation and GOS audits as directed. Performs other assignments as directed and is able to perform all duties in an efficient and effective manner to ensure that physician’s treatment plans are administered and monitored through patient care services provided on the rehabilitation unit. • Manages inpatient rehab unit data related to prospective payment reimbursement and patient outcomes. This includes collection of inpatient rehabilitation facility/patient assessment instrument (IRF/PAI) data, verification of its accuracy based on patient’s case mix group (CMG), and coordination with HIM coding to ensure records are coded on a timely basis, and maintaining transmission schedules to ensure CMS fiscal intermediary. • Ensures compliance with legal, regulatory and clinical policies and procedures. • Helps with yearly competency of FIM testing of staff and training of new employees • Works with the Medical Director to ensure documentation supports the IRF patient. • Inputs IRF-PAI data into UDS-MR and monitors the data from this site.
