The incumbent serves as a key member of PACT, partnering with other healthcare professionals to provide healthcare that is patient-centered, data-driven, team-based, and continuously improving.

Travel may be required from the assigned site to any of the other GLA sites as needed.

The duty location is at SANTA MARIA, CALIFORNIA.

Provides an extensive range of clinical pharmacy specialist services to patients in PACT. Works as a member of an assigned PACT team and monitors patients for a variety of disease states including diabetes, hypertension, etc. Designs, implements, assesses, monitors, and documents therapeutic plans using the most effective, least toxic, and most economical medications. Offers additional interventions to maximize patient care outcomes, taking into consideration choice of therapy, safety, efficacy, and cost.

Works closely with other healthcare professionals including physicians and nurse practitioners. Performs physical assessments of patients and orders laboratory tests to help assess the efficacy and any negative aspects of medication therapy.

Conducts and evaluates medication histories, assesses compliance and suggests pharmacotherapeutic modifications in an effort to achieve desired outcomes and provides appropriate pharmaceutical care.

Serves as an expert PACT pharmacist providing clinical pharmacy consultations to professional staff on the pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics of pharmaceutical products.

Identifies any drug-related problems such as allergic reactions, toxicities, adverse reactions, improper dose regimen, noncompliance, etc.

Provides medication counseling and educational materials to patients. Works with patients to achieve compliance with medication regimens.

Processes pending orders, as needed.

Identifies, interprets, and resolves prescribing and dispensing issues. Documents interventions when appropriate.

Monitors patients' anticoagulation therapy through telephone consultations or clinic visits at intervals determined by the pharmacist. At each follow-up encounter, reviews lab results and evaluates patients for any changes related to their anticoagulation therapy. As needed, makes dose changes or other changes to patients' anticoagulation therapy. Documents monitoring results and any medication changes in CPRS.

Program Management and Administrative Functions:

  • Establishesand maintains work flow patterns to keep outpatient prescription waiting time to an average of 30 minutes or less.
  • Checks inpatient and outpatient prescriptions/orders that are filled by pharmacy technicians ensuring their accuracy and completeness prior to dispensing/administration. Assists in identifying, distributing, and balancing workload and tasks among pharmacy technicians.
  • Effectively organizes workload, plans short and long range objectives, solves problems, and achieves goals within budget and staffing constraints.
  • Participates in quality assurance activities related to pharmacy issues.
  • Responsible for complying with all applicable Federal, VA, and GLA regulations, policies, and procedures pertaining to control substance accountabilty and drug accountabiltiy for pharmacy.
  • Complies with Joint Commission, CAP, CARE, and any other accreditation/regulatory requirements in connection with Pharmacy.
  • Ensures assigned administrative projects and reports are submitted in a timely fashion.
  • Uses discretion in appropriately safeguarding confidential information.
  • Effectively utilizes the VISTA/CPRS computer system as well as automated pharmacy equipment.


  • Provides teaching to pharmacy trainees including residents, pharmacy students and pharmacy technician students
  • Provides forman and informal education and inservices to pharmacy, medical, nursing and other healthcare staff.
  • Collaborates with Pharmacy Program Manager (Education and Training), to promote the learning experience and improve the performance of all levels of pharmacy staff.
  • Participates in all aspects of the training such as conducting on-the-job training and monitoring performance.
  • Develops and conducts patient education classes.
  • Works with researchers on protocol requirements and investigational record keeping. Assists in protocol development and implementation, as needed.

Tour of Duty: The employees in this position will work five (5) days a week and require to rotate for staffing coverage. Employees occasionally rotate Saturdays and holidays, and who work in the weekends and holidays, are entitled to receive weekend premium and holiday pay when worked.

Education: (COPY OF TRANSCRIPTS REQUIRED):(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm. D.) degree.

NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)

(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).

Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.

English Language Proficiency:Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).


GRADE DETERMINATIONS:In addition to the basic requirements, the following criteria must be met when determining the grade of candidate.

GS-12 Clinical Pharmacist (Full Performance Level)

Experience or Education.In addition to the basic requirements, candidates must meet one of the following:

  1. 1 year of experience equivalent to the next lower grade level, or

  2. Completion of an ACPE-accredited Pharm.D. program.

Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):

  1. Knowledge of professional pharmacy practice.

  2. Ability to communicate orally and in writing to both patients and health care staff.

  3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.

  4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
