OSF Healthcare Pediatric Diabetes -Social Worker II/Counselor in Peoria, Illinois
Reporting to the Manager, assists patients and their families in understanding illness and disability, and in resolving complex personal, financial and environmental difficulties which predispose illness and interfere with obtaining maximum benefits from medical care. Provides consultation to healthcare professionals regarding impact of psycho-social factors on diabetes. Provides appropriate psychosocial therapies to assist patients and families in coping with problems related to the patients’ medical care, using individual treatment methods. This position may provide mental health services for the evaluation, treatment and prevention of mental and emotional disorders in individuals, families and groups. Demonstrates the knowledge and skills necessary to provide patient services appropriate to the ages of the patients served.
The job incumbent is required to demonstrate the knowledge and skills necessary to provide patient care that is appropriate to the age of the patients served on their assigned unit(s).
This requires that the incumbent demonstrates knowledge of growth and development as well as the physical, emotional, and psych-social needs of the patient population served.
The completion of a Master’s Degree in Social Work from an accredited school of social work and the possession of a current State of Illinois license in social work is required.
Licensure as LCSW is preferred for this position.
Approximately 1 to 2 years progressively more responsible related work experience necessary in order to gain full knowledge of casework process, community resources and outside agencies in relation to health, illness and disability, and to be able to handle more complex cases.
Interpersonal skills necessary in order to interact effectively with Medical Center and outside agency healthcare professionals, with patients and families often during periods of stress, and to act as resource person for other Medical Center personnel.
Analytical skills necessary to assess patients' needs, develop advanced treatment and discharge planning programs, and provide effective counseling.
