The Indian Health Service (IHS) is the principal federal health care provider and health advocate for American Indians and Alaska Natives (AI/AN). The IHS goal is to raise Indian health status to the highest possible level. IHS provides leadership and direction to programs and activities designed to improve health outcomes to over 1.9 million AI/AN through a system of IHS, Tribal, and Urban operated facilities and programs.
This is an "Open Continuous Vacancy Announcement" and applications will be referred when vacancies occur. Applications will remain eligible for three (3) months from the time the application is received.
Position Details: May be Permanent, Temporary, or Term with a tour of duty of Full-Time, Part-Time or Intermittent. The Agency has the option of extending Term Appointments for up to a maximum of four years.

This position requires that you obtain and maintain clinical privileges. If they are not obtained or maintained during employment, the employee may be subject to adverse actions, up to and including removal from the Federal service.
Provides ambulatory medical home care team services with both medical provider staff and other health care staff.
Ensures patients receive quality patient centered care that is consistent with the patient-centered medical home principles, and care is timely.
Implements and initiates disease management and health promotion guidelines.
Develops a plan of care that identifies resources, integrates patient family education, evaluates patient care, and modifies the plan of care based on individual patient outcomes.
Participates, sets priorities, and identifies opportunities to improve nursing performance through data collection and monitoring activities.
Coordinates clinic processes, instructs employees on specific tasks/job techniques, evaluates the competency.

To qualify for this position, your resume must reflect sufficient experience and/or education, to perform the duties of the position for which you are applying. Education: Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant.
Registration: Applicants must have active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below:
Your resume must demonstrate at least one year of specialized experience at or equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector.
GS-10 Grade Level:
- 1 year of specialized experience equivalent to at least the GS-09 grade level.
At GS-09 and above, positions require 1 year of specialized experience in the specific specialty area of nursing that is being filled.
Example of Specialized Experience: Experience on a care team where applicant is responsible for chronic disease management of empanelled patients, leads the care team in day to day population based management and proactive planning for chronic and preventive disease screening.
Note: Equivalent combinations of education and experience are qualifying for all grade levels and positions which both education and experience are acceptable. Graduate-level education must have been in nursing with a concentration in a field of nursing (e.g., teaching, a clinical specialty, research, administration, etc.) or in a closely related non-nursing field directly applicable to the requirements of the position to be filled.

CONDITIONS OF EMPLOYMENT:
Measles, Mumps and Rubella (MMR) immunization required for selectees born after 1957.
